M.I.T.
*Rules of Engagement
For consideration to participate in M.I.T. the client agrees and understands:
- M.I.T. is a non church-based ministry lead by Jerry Carmody. Jerry provides professional coaching and training in individual and group settings. Jerry is not licensed as a professional counselor, social worker, or psychologist and does not offer psychological counseling. If these needs arise, a professional counselor should be consulted.
- Contacts with M.I.T client will ideally be in person, however from time to time extenuating circumstances may require a phone meeting. Face to face contact is typically required for the first two (2) meetings and should allow enough time during a meeting for discussion of goals, as well as questions from the client concerning their professional and/or personal development. Typically there will be two early morning meetings a month for 1.5 hours for each meeting held in Westlake. Phone follow up calls are typical and expected.
- As professional coach and client, we agree to keep our discussions in confidence. However, you should be aware there are some situations in which I may be required by law to report information to the proper authorities without your permission or knowledge. These situations include, but may not be limited to: an intent of harm to self or others, involvement in a felony, suicidal intentions, and/or reasonable evidence of child, disabled, or elder abuse or neglect. A disclosure of information may be required in response to a subpoena and/or court order issued by a court of law.
- Client agrees that by signing this agreement, they are consenting to participate in the Men in Transition Ministry and understand it is not a promise of any job, specific introduction or outcome. It too is understood that client may terminate their participation in M.I.T. at any time.
- This agreement herein referred to as the “M.I.T. Agreement,” will govern all relations involved during the term of the M.I.T. ministry process. It is agreed that any disputes or modifications of this M.I.T. Agreement will be determined directly between the individuals involved.
